Patient Health Assessment

Doctors from Stanford Clinic join The Marfan Foundation’s Professional Advisory Board doctors to give this extraordinary opportunity to registered conference attendees. Conference attendees who don’t have access to quality medical care in their home area due to lack of knowledgeable doctors and/or lack of healthcare coverage can apply for a free patient health assessment. Appointments are for people who are seeking a diagnosis or have a specific Marfan or related disorder treatment issue. Patient assessments take place on Thursday, July 12, and Friday July 13, 2018. Please complete a separate application for each person requesting an appointment. Each application must be submitted separately.

All echocardiogram and eye appointments are on Thursday, July 12, 2018. All other appointments are on Friday, July 13, 2018. If you don’t need an echocardiogram or eye appointment, but need to see the other specialists, assume you have an appointment on Friday, July 13, 2018. Please make travel arrangements assuming you have the first appointment of the day.

Adults/children must be registered for the conference to apply for a free patient health assessment. Applications must be received no later than April 15, 2018. 

Applications for first time attendees to the health fair will receive priority in addition to other criteria. Completing and submitting the application does not guarantee an appointment.

Step 2: Upload and Submit Your Completed Application

Once you have completed the application for yourself or your child and saved it to your computer, please click on “Upload” and attach your completed application and submit. We strongly suggest that you complete the application electronically in order to ensure legibility. If you cannot complete the application electronically, you can complete the application by hand and then fax or mail your application as indicated on the application form.
