The intent of the scholarship program is to sponsor people who, due to financial hardship, would not otherwise be able to attend. In an effort to assist as many people as possible, it is highly unlikely that those who have attended a previous conference or received a scholarship in the past would be awarded a scholarship this year.

1. Complete the application form and return by May 15^th

            All information in the application is strictly confidential.

1. All applicants for scholarship must also register for conference using either the paper registration form found in the NMF conference brochure or by registering online at the NMF website www.marfan.org (from the Home page click on the “Conference & Events” tab)

1. If you need to be seen by a physician for a patient assessment, you must make an appointment PRIOR to receiving your scholarship notification. The NMF is not responsible for making appointments on your behalf. The deadline for patient assessment appointments is Friday, May 15, 2009.

We are planning to have letters in the mail on or about June 1, 2009.

Upon learning that you have received a scholarship, you will be required to either accept or decline the award.
