Registration:

What meals are included with my registration?

Full Conference attendees:

-Friday Welcome Reception (Light Fare)

-Saturday and Sunday continental breakfast and lunch

One-day only:

-Breakfast and lunch on the day of attendance/day you are registered for

Children Under 5 Years Old:

Do I need to register my child who is under 5 years old for the Conference?

No, children under 5 do not need to register for the Conference. Children under 5 may apply for a patient assessment if a
parent or guardian is registered for the Conference. However, there is no programming throughout the Conference for children under 5 years old.

I am bringing my child who is under 5 years old. Are there activities for this age group?

Unfortunately, there are no activities for children under 5 years old although they may apply for a patient assessment at the Conference Health Fair.

Is childcare provided for children under 5 years old at the Conference?

Unfortunately no. You will need to provide your own childcare for the Conference. Please do not bring children under 5 years old to the adult program activities such as the general session and/or workshops. Bringing a young child to adult activities is disruptive to other Conference attendees and unfair to the child(ren). In the past some families with two parents/guardians on site have chosen to trade off providing childcare. While one parent is attending the Conference the other is providing childcare. If you choose to share the childcare, then it is only necessary to register and pay for one adult for the Conference. Some families choose to bring an extra adult to the Conference such as a grandparent, other relative, or friend to watch young children. If you choose this option, the additional adult does not need to register for the Conference since they won't be attending Conference activities
however if they will participate in Conference meals additional meal tickets will need to be purchased. Often hotels are able to provide childcare for a fee or to provide suggestions.

Can my child who is under 5 years old participate in the Conference Health fair/apply for a patient assessment?

Yes, children under 5 years old can participate in the Health Fair and apply for a patient assessment as long as they are accompanied by an adult who is registered for the Conference.

Patient Assessments at the Conference Health Fair:

I need to make hotel reservations, but don’t know when my patient assessment appointment(s) is scheduled yet? Is
there any information that could help me plan my arrival date
/hotel stay?

All echocardiogram and eye appointments will be on Thursday August 1, 2013 beginning at 8:00 am. If you will need an echocardiogram or eye appointment or both make your plans assuming you will need to be on site at the Conference for a Thursday, August 1, 2013 appointment at 8:00 am. All other patient assessments will take place on Friday, August 2nd, 2013 beginning at 8:00 am (cardiology/heart, cardiac surgery, genetics, pulmonary/lungs, orthopedics/bones and joints, dental, etc.). If you will require a patient assessment with any specialist other then an eye specialist make your plans assuming you will
need to be on site at the Conference for a Friday, August 2nd, 2013 appointment at 8:00 am.

How and when will I be informed about my patient assessment appointment  if I applied for one?

You will receive an email directly from the hospital(s) hosting the Conference Health Fair at the end of June.

Conference Scholarship:

Who is eligible to apply for scholarship?

Anyone 19 years old or older who resides in the USA and has not attended the NMF Conference in the past and/or has not received a past NMF Conference scholarship. Children and teens are always free with a registered adult.

Can those 18 and younger apply?

The parent or guardian is the person who applies and receives the scholarship. Children and teens may attend for free provided they are accompanied to the Conference by at least one paying or scholarship adult.

What expenses does the scholarship program cover?

Typically, one adult registration and two hotel nights. However, this can change year to year based on number of applicants and funds raised for scholarship that year.

Does the scholarship cover transportation costs?

No, only registration and hotel.

If I apply for a scholarship do I register at the same time?

No, wait until you receive the scholarship decision letter before registering for the conference.

If my scholarship application includes a request for a hotel room should I book the hotel?

No, the NMF has a block of rooms reserved for this purpose. DO NOT make the reservation yourself, we will make it for you and inform you of the arrangement about a month before Conference.

If I am awarded a scholarship what do I need to do next?

First, fill in the Scholarship Response form that was emailed to you with your award letter. Second, register for the Conference. Third, if you or one of your family members is planning to apply for a patient assessment at the Conference Health Fair, complete the patient assessment application. If you are unable to accept your scholarship, please return the Scholarship Response
Form and check the box “I am declining my scholarship."
